    Week 3
    Hello everyone,
This week, you have your first mind map assignment. The focus will be cardiovascular
disorders. Within the assignment, you will see a list of disorders to choose from. You will
pick one topic and create a mind map. You can use any template for the mind map and
some examples have been provided for you in this week's learning resources. Some key
points to remember when completing this assignment:
    1. Ensure that you follow the rubric and address all sections.
    2. Once you complete the mind map and submit the assignment, it is your responsibility to
ensure that I can open it and read it clearly. Make sure that the text is large enough and it
does not link to a site needing a log in. If I can't read the text or access the assignment, I will
add a comment in the grading section and it will need to be resubmitted. The resubmission
will have the grade deducted per policy.
    3. This assignment must be in the form of a mindmap. If it is turned in as an essay, it will not
be graded and you will receive a comment to resubmit as a mindmap.

    Clinical Skills Self-Assessment

![](https://www.sweetstudy.com/questions/clinical-skill-self-assessment)Before embarking on any professional or academic activity, it is important to understand the background, knowledge, and experience you bring to it. You might ask yourself, “What do I already know? What do I need to know? And what do I want to know?” This critical self-reflection is especially important for developing clinical skills such as those for advanced practice nursing.

Specifically, you will identify strengths and opportunities for improvement regarding the required practicum skills. In this practicum experience, when developing your goals and objectives, be sure to keep assessment and diagnostic reasoning in mind.

You might ask yourself, “What do I already know? What do I need to know? And what do I want to know?” This critical self-reflection is especially important for developing clinical skills such as those for advanced practice nursing.

The Psychiatric-Mental Health Nurse Practitioner (PMHNP) Clinical Skills List and Clinical Skills Self-Assessment Form provided in the Learning Resources can be used to celebrate your progress throughout your practicum and identify skills gaps. The list covers all necessary skills you should demonstrate during your practicum experiences.

Just as you have in previous practicum courses, for this Assignment, you assess where you are now in your clinical skill development and make plans for this practicum. Specifically, you will identify strengths and opportunities for improvement regarding the required practicum skills. In this practicum experience, when developing your goals and objectives, be sure to keep assessment and diagnostic reasoning in mind.

    – **Assignment Preparation**:

o    Review this week’s Learning Resources. Consider the insights provided about assessing and diagnosing anxiety, obsessive compulsive, and trauma-related disorders.

o    **Review the Focused SOAP Note template**, which you will use to complete this Assignment. _**There is also a Focused SOAP Note Exemplar provided as a guide for Assignment expectations.**_

o    Review the video, _Case Study: Dev Cordoba_. You will use this case as the basis of this Assignment. In this video, a Walden faculty member is assessing a mock patient. The patient will be represented onscreen as an avatar.

o    Consider what history would be necessary to collect from this patient…. _**Think about a comprehensive assessment and compare the information**_

o    Consider what interview questions you would need to ask this patient….

_**The Assignment.  Please use formal sentences to respond to questions.  You can utilize these headings to keep it simple and to ensure you are answering the required content.  Remember you must validate when necessary to justify your response with resources dated from 2020-2025.  If it is a required reading for the course, it can be utilized.  However, majority of your support should come from peer reviewed/scholarly articles.**_

    _**Subjective:**_

What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis?

What is the duration and severity of their symptoms?

How are their symptoms impacting their functioning in life?

    _**Objective:**_

What observations did you make during the psychiatric assessment?

    _**Assessment:**_

Discuss the patient’s mental status examination results.

    What were your differential diagnoses?

***Provide a _**minimum of three possible diagnoses with supporting evidence**_, listed in order from highest priority to lowest priority.

Compare the _DSM-5-TR_ diagnostic criteria for each differential diagnosis and explain what _DSM-5_-TR criteria rules out the differential diagnosis to find an accurate diagnosis.

Explain the critical-thinking process that led you to the primary diagnosis you selected.

Include pertinent positives and pertinent negatives for the specific patient case.

**Plan:** _**Be sure to provide an evidence-based rationale to support your therapeutic reasoning.**_

What is your plan for psychotherapy?

What is your plan for treatment and management, including alternative therapies? Include _**pharmacologic and nonpharmacologic treatments**_, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan.

Also incorporate one health promotion activity and one patient education strategy.

**Reflection notes:** _**Be sure to provide an evidence-based rationale to support your therapeutic reasoning.**_

What would you do differently with this patient if you could conduct the session again?

Discuss what your next intervention would be if you could follow up with this patient.

Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion, and disease prevention, taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).

_**Provide at least three evidence-based, peer-reviewed journal articles or evidenced-based guidelines that relate to this case to support your diagnostics and differential diagnoses. Be sure they are current (no more than 5 years old….2020-2025).**_

Manage your time…You have the KEY to SUCCESS!!!
