The Effects of Antipsychotic Medication on Cognitive Decline in Nursing Home Residents
Background:
Antipsychotic medications are frequently prescribed to nursing home residents for behavioral and psychological symptoms of dementia, but their association with subsequent cognitive decline remains uncertain. Using the Resident Assessment Instrument–Minimum Data Set 2.0 (RAI-MDS 2.0), this study examines whether antipsychotic exposure is associated with accelerated cognitive deterioration among long-term care residents.
Objective:
To evaluate the relationship between antipsychotic medication use and longitudinal change in cognitive status among nursing home residents documented in the RAI-MDS 2.0 dataset.
Methods:
We conducted a retrospective cohort analysis of residents captured in RAI-MDS 2.0 assessments. Baseline characteristics, medication records, and serial cognitive performance measures were extracted. Exposure was defined as receipt of antipsychotic medication during the observation period; comparisons were made between exposed and unexposed residents. Multivariable mixed-effects models adjusted for demographic factors, baseline cognition, comorbidities, markers of dementia severity, behavioral symptom burden, and facility-level clustering. Sensitivity analyses included propensity score adjustment and stratification by typical versus atypical agents and by indication (psychosis vs. behavioral symptoms).
Results:
After adjustment for confounders, antipsychotic use was associated with a statistically significant greater rate of decline in standardized cognitive scores compared with nonuse. The magnitude of decline varied by analytic approach but remained consistent in propensity-adjusted models. Residents receiving antipsychotics for nonpsychotic behavioral symptoms experienced a similar or greater association with cognitive decline compared to those treated for psychosis. Results were robust to exclusion of short-term users and to alternative model specifications.
Conclusions:
In this RAI-MDS 2.0–based analysis, antipsychotic medication exposure was associated with accelerated cognitive decline among nursing home residents. These findings support careful consideration of risks and benefits when initiating antipsychotics in long-term care, emphasize the importance of regular medication review, and reinforce the need for nonpharmacologic strategies and close monitoring of cognitive trajectories. Future prospective studies are warranted to clarify causal pathways and to identify subgroups at heightened risk. -
